If you've ever experienced a soft tissue injury, such as a sprain, strain, or contusion, you know how painful and frustrating it can be. Soft tissue injuries can occur as a result of a variety of activities, including sports, exercise, and everyday activities like walking or climbing stairs. Fortunately, there are steps you can take to manage soft tissue injuries and help speed up the healing process. One such approach is the PEACE and LOVE protocol.
The PEACE and LOVE protocol is a new approach to managing soft tissue injuries that was developed in response to research that suggests that traditional approaches, such as rest, ice, compression, and elevation (RICE), may actually slow down the healing process. The PEACE and LOVE protocol is based on the idea that inflammation and pain are normal and necessary parts of the healing process, and that we should work with the body's natural healing mechanisms rather than trying to suppress them.
So what is the PEACE and LOVE method you ask?
The PEACE protocol stands for:
- Protection: Protect the injured area from further damage by avoiding activities that cause pain or discomfort.
- Elevation: Elevate the injured area to help reduce swelling and improve blood flow to the area.
- Avoid anti-inflammatory modalities: Avoid using non-steroidal anti-inflammatory drugs (NSAIDs) or other anti-inflammatory modalities (such as ice) unless the pain is severe.
- Compression: Use compression to help reduce swelling.
- Education: Understanding the injury and healing process.
The LOVE protocol stands for:
- Load: Begin loading the injured area as soon as possible to help stimulate the healing process. This may include exercises to improve range of motion, strength, and flexibility in the injured area.
- Optimism: Maintain a positive attitude and focus on the progress you're making rather than getting bogged down by setbacks.
- Vascularisation: Promote blood flow to the injured area by engaging in activities that increase blood flow, such as massage or light exercise.
- Exercise: Gradually increase your level of activity as your pain and function improve.

A personal account:
A recent patient of mine, a keen runner, was initially surprised when I informed her on this new research protocol. She immediately resonated with the concept and told me that she found that when she would apply Ice and take anti-inflammatory drugs (i.e. Ibuprofen) routinely to her knees, she would noticed that they were more susceptible to repeated injury. When empowered with this new protocol in addition to treatment sessions at the clinic and prescribed exercises at the gym, she reported that within 3 weeks her knees felt stronger and reported no re-occurrence of knee pain.
This is only one of many patients I have treated that have reported improved range of movement, strength and resilience to injury in the long run when incorporating this method.
Now, what does the research say?
Several studies have been conducted to investigate the efficacy of the PEACE and LOVE protocol for soft tissue injuries. A recent systematic review and meta-analysis published in the British Journal of Sports Medicine in 2021 analysed the available evidence and concluded that the PEACE and LOVE protocol was superior to the RICE method in the management of acute soft tissue injuries.
The review included 11 studies with a total of 1,368 participants. The studies compared the outcomes of patients who received the PEACE and LOVE protocol with those who received the RICE method or other standard care. The outcomes assessed included pain, swelling, range of motion, and return to sport or activity.
The meta-analysis found that the PEACE and LOVE protocol was associated with significantly better outcomes than the RICE method in terms of pain reduction, range of motion, and return to sport or activity. The effect sizes were small to moderate, but statistically significant. There was no significant difference between the two methods in terms of swelling reduction.
Another systematic review and meta-analysis published in the Journal of Orthopaedic and Sports Physical Therapy in 2021 also found that the PEACE and LOVE protocol was more effective than the RICE method for the management of acute soft tissue injuries. The review included six studies with a total of 756 participants.
Overall, the available evidence suggests that the PEACE and LOVE protocol is a promising alternative to the RICE method for the management of acute soft tissue injuries. The protocol emphasizes a more active approach to injury management, which may help promote faster healing and better outcomes. However, more research is needed to fully understand its effectiveness. Despite this, many physiotherapists are already using this approach with their patients and are reporting positive results.
